Based on an online licensing application that has been submitted and accompanied by data/information/description, attachments and explanations in the Application form online (herinafter referred to as data), then I hereby declare the following:

1. Provide data in physically and mentally healthy condition and not under duress.
2. Willing to understand and comply with the terms and conditions that had been determined for the use of the Online Licensing Application System of The Ministry of Energy and Mineral resources.
3. All documents and information related to the company profile uploaded in this system are appropriate, correct and up to date.
4. The account of the business entity that has been created is a representative account of the business entity and made by the officer who has been given power by the business entity.
5. All documents and information uploaded to this system are true and valid and accountable for the truth and its validity, if found to be inappropriate or incorrect documents then the applicant is ready to be responsible in accordance with the prevailing laws and regulations.
6. Any documents issued by the Agency requested by application with this data (including but not limited to information, description, explanation, letter, certificate, Decree, Letter of consent), do not give the right, authority, recommendation, any approval except what is actually mentioned in the certificate, Decree, Letter of approval and/or in any form provided by the agency and can be proved in the recognized manner Regulations that apply as a result/result of data-driven applications.
